A TSH just outside the range is common, often temporary and seldom a reason to start medication that week.

Thyroid results cause more needless worry than almost any other panel. TSH is sensitive: it shifts with illness, stress, pregnancy, the time of day and the biotin in a hair supplement.

One reading is not a diagnosis

A TSH of 5.2 with a normal free T4 and no antibodies usually calls for a repeat in six to eight weeks, not a prescription. About half of those readings return to normal on their own.

When it does matter

A TSH that stays high with a low free T4, or raised TPO antibodies, is a different story: that pattern is worth acting on, and worth an endocrinologist’s look.

Either way we run the whole panel, because TSH on its own can’t tell those two stories apart.
